Insurance Panels
This list is not a guarantee that your insurer will pay for your treatment. Furthermore, I may be on some panels not listed here. Please verify with your insurance company directly. You are responsible for obtaining preauthorization for your first session if your insurer requires it.
***Optum, Providence, and non-Nike-sponsored United (or any other plans not listed) members*** Although I am not in-network with your insurance, you are welcome to use your out-of-network option to see me. I can not access benefit information online as an out-of-network provider, so you will need to check on your deductible and/or copay and let me know, but I will still take care of all of the billing so you don't have to deal with that hassle. Usually the deductible and copay are higher than your in-network options and your out-of-pocket maximum may be different as well.

Fees
Payment of your copay, deductible or session fee (if you are using out-of-network coverage or not using insurance) is due at the time of service. I accept cash, check, Visa, Mastercard, American Express, and Discover. I charge $200 for an intake session and $180 per hour thereafter with a $20 per session discount for self-paying clients.
***Please keep in mind that many plans have deductibles which need to be met at the beginning of each plan year (usually the calendar year but check with your insurer). This means that your insurance will not start paying until you have paid the deductible amount out of pocket so you will be responsible for the full session fee allowable by your insurance rather than just the copay. The full session fee may vary from the amounts noted above because different insurance companies have different contracted reimbursement rates. Please check with me if you need assistance determining how many sessions it will take to meet your deductible.
